This form is for the proposed transferee already holds the same type of salmon permit (area and gear) except Cook Inlet set gillnet, Cook Inlet drift gillnet and Yakutat set gillnet, otherwise use the Request for Permanent Transfer of Entry Permit form.

If any of the information is not submitted, or if the form is incomplete, there will be delays in processing the request.
